WASHINGTON - Peg Perego is recalling hundreds of thousands of strollers because children can become trapped and strangled between trays on them. One death has already been reported.
  
Peg Perego USA Inc. said Tuesday that a six-month-old died of strangulation in a stroller in 2004, while a seven-month-old was nearly strangled in 2006. The company is recalling approximately 223,000 strollers, which include Venezia and Pliko-P3 strollers made between January 2004 and September 2007.
  
Only strollers with a child tray and one cup holder are part of the recall.
  
Separately, Kolcraft Enterprises Inc. is recalling some strollers due to potential falling and choking hazards.
  
Kolcraft is recalling about 5,600 of the Contours Options LT tandem strollers sold between February and July. However, no injuries have been reported.

Innovage Recalls Discovery Kids Lamps Due to Fire and Burn Hazards

Name of Product: Discovery Kids™ Animated Marine and Safari Lamps
Units: About 300,000
Importer: Innovage LLC, of Foothill Ranch, Calif.
Hazard: The placement of internal wires near the circuit board can cause electrical short-circuiting and sparking, posing a fire and a burn hazard to consumers.
Incidents/Injuries: Innovage has received 11 reports of short circuiting. This includes three reports of lamps catching fire, which led to property damage. No injuries have been reported.
Description: This recall involves Discovery Kids Animated Marine and Safari Lamps that feature rotating films with marine or safari scenes. The words "Discovery Kids" are printed on the front top-left corner of the product. The recalled products have both an 11-digit batch number that begins with either 584894 or 10128 and a model number of 1628626, 1642433, 1641522, 1641523, 1645729, or 1645853. Batch numbers can be found imprinted in the plastic underneath the lamps and on the bottom of the packaging. Model numbers can be found on stickers placed underneath the lamps and on the bottom of the packaging near the barcode.
Sold at: Bed Bath & Beyond, Bonton, JCPenney, Kohls, Office Max, Toys "R" Us stores nationwide as well as the online retailers Amazon, Ideeli, JCPenney, Kohls, Macy's and Overstock from June 2010 through March 2012 for between $10 and $20.

Dole Recalls Salad Sold in South Carolina | wltx.com

 WLTX)- Dole Fresh Vegetables is voluntarily recalling 2,598 cases of bagged salad.
The product being recalled is Dole Hearts of Romaine coded 0540N165112A or B, with Use-by date of June 26 and UPC 7143000956 due to a possible health risk from Listeria monocytogenes.
Dole Fresh Vegetables is coordinating closely with regulatory officials.
No illnesses have been reported in association with the recall.
The product code and Use-by date are in the upper right-hand corner of the package; the UPC code is on the back of the package, below the barcode.
The salads were distributed in nine U.S. states (Alabama, Florida, Georgia, Maryland, North Carolina, Pennsylvania, South Carolina, Tennessee and Virginia).
No illnesses have been reported in association with the recall.
This precautionary recall notification is being issued due to an isolated instance in which a sample of Dole Hearts of Romaine salad yielded a positive result for Listeria monocytogenes in a random sample test conducted by the FDA.
No other salads are included in the recall.
Only the specific Product Codes, UPC codes and June 26, 2012 Use-by date identified above are included in the recall.
Consumers who have any remaining product with these Product Codes should not consume it, but rather discard it.

Lack of lot codes, records hamper sprout recall - The Packer

State officials said they are having difficulty tracking what retailers and foodservice operations received the sprouts because the packaging does not have lot codes or other traceable labeling and the company does not have complete records about their distribution. Government food safety officials say a listeria-related sprout recall by Henry’s Farm Inc. is hampered because the sprouts were distributed without lot codes or other traceable labeling and the company's records are incomplete.
As of May 3, no illnesses had been reported, but it was not known what quantities of the Spring Water Soybean Sprouts and Spring Water Natto Soybean Sprouts had been distributed.
Investigators were also unsure who bought the sprouts because of inadequate records. They believe ethnic markets and restaurants are the primary customers of Soo Park, the owner of Henry’s Farm, Woodford, Va.
“After reviewing records, we don’t feel we can give an accurate estimate on the volume that has been distributed – and certainly not a definitive answer,” said Elaine Lidholm, director of communications for the Virginia agriculture department.
“His records are incomplete and his labeling is inadequate so it’s difficult to determine how much product he moves. FDA and (our department) will determine a course of regulatory action. If he continues production, we can observe his process from beginning to end first hand and know from that point on how much he is distributing and to whom.
The initial April 27 recall involved sprouts sent to Virginia, Maryland and Washington, D.C. Test samples taken by the Virginia Department of Agriculture and Consumer Safety showed positive results for listeria monocytogenes.
Henry’s Farm expanded the recall April 30 to include the natto sprouts. Calls to the company did not go through because its voicemail system was full and not accepting messages.
Lidholm said inspectors from the department and the Food and Drug Administration were at Henry’s Farm the week of April 30 as part of a continuing investigation. She said an interpreter might have to be called in to communicate with the owner.
The initial test samples came from product displayed at retail on April 17.
“This is gonna be a hard one since we don’t know how many were distributed initially due to no lot coding,” Lifholm said. “We’re going after this hot and heavy.”
The soybean sprouts are in 1.5-pound clear plastic packages and 10-pound plastic bags; the natto soybean sprouts are in 1-pound packages.
The FDA issued a recall notice on the sprouts that was dated April 30, but it was not posted on the agency’s website or sent to alert subscribers until May 2.

Hazard: A child can place his or her fingers in the opening at the grab bar's hinge mechanism. When the consumer lifts the grab bar to remove the child from the seat, the child's fingertips can be caught in the hinge mechanism, posing a laceration and fingertip amputation hazard to the child.
Incidents/Injuries: The firm has received two reports of incidents including near amputations that resulted in stitches and a crushed finger.
Description: This recall involves Topeak Babyseat II bicycle carrier seats with model numbers TCS2100, TCS2101 and TCS2102. Model numbers are printed on the product's packaging. The gray, plastic Babyseats were sold in three styles: Babyseat, Babyseat with disc brake compatible rack and Babyseat with non-disc brake compatible rack. The racks are used to mount the seat to the bicycle. "Topeak" is embossed on the back of the seat and is also printed on a black foam protector that covers the grab bar. A red locking lever on the side of the blue grab bar locks the bar into place.
Sold at: J&B Imports, REI, Action and Hawley, independent bicycle dealers, distributors and other retail stores nationwide and online at REI.com from January 2009 through April 2012 for between $140 and $180.

Safety 1st Cabinet Locks Recalled Due to Lock Failure; Children Can Gain Unintended Access to Dangerous Items

Hazard: Young children can disengage the cabinet locks, allowing access to cabinet contents and posing the risk of injury, due to dangerous or unsafe items.
Incidents/Injuries: DJG has received 200 reports of locks that did not adequately secure the cabinet, including reports of damaged locks. Of the reported incidents, the firm is aware of 140 children between the ages of 9 months and 5 years who were able to disengage the locks and gain access to the cabinet’s contents. In three of the reported incidents, the children who gained access swallowed or handled dishwashing detergent, window cleaner or oven cleaner, and were treated, observed and released from emergency treatment centers.
Description: This recall involves Safety 1st Push ‘N Snap cabinet locks with model numbers 48391 and 48442. The model numbers are printed on the back of the product and on packaging. The locks are used to secure cabinets with two straps that wrap around the knobs or handles on a cabinet door. When the product is in the “lock” position, a green triangle is shown through a window on the device. The Safety 1st logo is embossed on the front of the lock. Locks manufactured between January 2004 and November 2010 are included in the recall. The date of manufacture is embossed on the back. The arrow on the date wheel points to the month and the numbers of either side of arrow represent the year of manufacture.

Michael Foods widens egg recall - State & Regional - Wire - TheState.com

MINNETONKA, Minn. — A Minnesota food company is widening its recall of hard-cooked eggs because of a potential for listeria contamination.

Michael Foods, of Minnetonka, is recalling eggs in brine sold in 10- and 25-pound pails for institutional use in 34 states. The eggs carry six brand names: Columbia Valley Farms, GFS, Glenview Farms, Papetti's, Silverbrook and Wholesome Farms.

The U.S. Food and Drug Administration says the eggs were produced at Michael Foods' plant in Wakefield, Neb., and were bought by food distributors and manufacturers. There have been no reports of illness connected to the eggs. But, the company says the eggs have the potential to be contaminated with a strain of listeria that can cause serious infections in those with weaker immune systems.
